Analysis Plan TitleDeterminants of COVID-19 Vaccine Hesitancy Among Pregnant Women: Evidence from the iCARE Study 
Analysis Plan DescriptionThe proposed research will investigate COVID-19 vaccine hesitancy during pregnancy across four countries—Italy, France, Ireland, and Canada. Despite well-established benefits of maternal immunization, uptake of COVID-19 vaccines among pregnant women remains suboptimal, influenced by unique behavioral, social, and contextual factors. This study aims to: (1) characterize the prevalence and patterns of vaccine hesitancy in pregnancy across the selected countries; and (2) examine associations between vaccine hesitancy and sociodemographic, behavioral, and health-related variables. The analysis will focus on pregnant women aged 18 and above from the most recent representative web-panel samples within the iCARE dataset. Descriptive statistics will summarize participant characteristics, including sociodemographic (e.g., country, age, education, income) and health-related variables (e.g., comorbidities, flu vaccination history). Motivators and barriers to COVID-19 vaccination, as well as trust in various information sources, will also be described. Bivariate analyses will explore relationships between hesitancy and key variables, followed by multivariate binary logistic regression to identify independent predictors of hesitancy. Insights into COVID-19 vaccine uptake during pregnancy can inform targeted strategies to increase immunization coverage and support acceptance of other maternal vaccines, such as pertussis and influenza, by addressing shared behavioral drivers.
